Task Purpose

With the increasing prevalence of chronic disease in Australia, adopting a person-centred and best-practice approach to nursing care is essential. This task will enhance your knowledge, skills, and awareness of self-management principles for chronic illness. You will analyse chronic disease models of care, the nurse’s role in interprofessional collaboration, and development of self-management goals using the selected case study.
