Assessment Task 4

You must demonstrate:

• Use and/or develop evaluation and monitoring, tools and technology
• Document and communicate outcomes to report on efficiency targets to key personnel and stakeholders
• Evaluate strategies and improvement plans
• Set new efficiency targets, and investigate and apply new tools and strategies
• Promote successful strategies and reward participants where possible

Task 4 (a)
Evaluation and monitoring:
Design a monitoring tool that could be completed each month to track the progress of the implementation plan. Use the table below to provide information.
You must use the same three resources you have used in task 3.

Task 4 (b)
Communication plan:
Document and communicate outcomes to key personnel and stakeholders. You may use the table provided.
